Very CommonMetformin (used for type 2 diabetes and weight management) is a well-established cause of B12 depletion — the longer you take it, the more it reduces your B12 levels. Proton pump inhibitors like omeprazole and lansoprazole, taken for acid reflux or heartburn, reduce the stomach acid needed to absorb B12. Many people on these medicines have never been told about this risk.
Medication-RelatedB12 requires a healthy gut to be absorbed. Crohn’s disease, coeliac disease, and bariatric (weight-loss) surgery all damage or bypass the part of the gut that absorbs B12. For these patients, oral supplements simply do not work — the gut cannot absorb them. An injection that bypasses digestion entirely is the clinically recommended solution.
